This role is responsible for providing accurate, updated, complete and reliable medical documentation to ensure quality patient care and facilitate delivery of health care services. The Medical Transcriptionist is responsible for transcribing clinical information into a printed format, the record of a patient’s medical history, assessment, diagnosis, etc according to the report completed by the Physician. The Medical Transcriptionist facilitates the flow of information between one health care professional to the next, providing high-quality patient health care reports by identifying any inconsistencies or inaccuracies in them.
Key Responsibilities:
1. Interprets and transcribes and types reports in the prescribed format in a timely manner
2. Transcribing and comprehending medical terminology in technical reports, including grammatical terms, punctuations, with a specified time period
3. Identify inconsistences, missing information or errors in the medical reports that can compromise patient care and coordinate with physicians for corrections
4. Proof read and edit reports making sure that the transcription is accurate, updated and complete.
5. Review and edit medical reports prepared by health care professionals or other documents prepared using speech recognition technology
6. Submit medical reports to physicians for approval
7. Follow patient confidentiality guidelines and legal requirements
8. Log and maintain record of medical reports transcribed
9. Translate medical jargon and abbreviations into proper form.
10. Identify and use appropriate medical and transcription reference resources as needed
11. Ensure accurate documentation of patient diagnosis, treatment and condition
12. Preparing and submitting activity/performance report monthly
13. Update resource materials regularly
14. Monitor and maintain equipment used for transcription
15. Verify accuracy of the dictator in identifying the patient by name, hospital number, location in hospital and any other pertinent information regarding the patient and is responsible for entering this information on prescribed form
16. Notify supervisor of problems with equipment, dictation or handwritten reports.
17. Complies with policies and procedures, and in particular infection control, safety and quality management.
18. Cover other Transcriptionists in different areas in the Department, in Secretary’s position and Supervisor’s post in their absence or vacations.
19. Perform other duties as assigned
